This PDF Pattern uses worsted weight (size 4 on the label) yarn, and the hook size is dependant on the desired size, in addition to a 4.0mm for detailing.
Baby - 3.5mm
Child - 4.0mm
Adult Small - 4.5mm
Adult Large - 5.0mm
Adult Extra Large - 5.5mm  
The Baby and Child sizes have slight changes to the pattern, and all instructions are labeled for size.
